DOCUMENTS TO BE UPLOADED INTO YOUR REGISTRATION APPLICATION

- One current proof of residency (xcel, lease, internet, water (something that pertains to the house)
- Student's birth certificate
- Current immunization records
- Parent/guardian photo ID, (If living with someone in our school boundaries, you will need a declaration form, and their ID) - use this link for the DECLARATION OF RESIDENCE form
